Care record unification for NHS Scotland using trusted data exchange
One unified care record for every social-care recipient.
The Challenge
People receiving social care — in care homes and community settings — accumulate records scattered across multiple systems held by different providers, health boards and local services. For the clinicians and care staff supporting them, no single system shows the whole person: information is duplicated, inconsistent or simply invisible at the point of care, and the burden of reconciling it falls on front-line staff.
The Solution
In this engagement for the NHS National Waiting Times Centre Board (a Special NHS Board in NHS Scotland, also known as NHS Golden Jubilee), NquiringMinds applied its trusted-data-exchange capabilities to support secure record unification for care recipients — bringing together records held across different systems into a single, more usable view while preserving the security and privacy controls that health and social-care data demands.
Outcomes
The short contract (April–July 2021) delivered a record-unification engagement for social-care recipients, extending NQM’s care-data portfolio alongside Care Analytics and CareShare.
TODO: capture specific outcomes, the care settings covered and any follow-on work.

Features
Unification of social-care recipients' records from multiple source systems into a single, usable view.
Secure, trusted exchange of sensitive care records between organisations.
Delivered as a short, fixed-scope engagement for an NHS Scotland Special Board.

Benefits
Delivered a record-unification engagement giving a more complete view of social-care recipients for the NHS National Waiting Times Centre Board (delivered engagement).
Reduced record fragmentation across care systems for the covered cohort (likely impact; quantification not recorded).
